Please complete and submit the completed form below. By submission of this form you are agreeing to hire Kina's Kreations/Kinatown.com to create favors/wrappers for your upcoming event. Please note that no refunds are granted (certain exceptions can be at the descretion by Kina only.
Submission of $50 deposit will reserve your party date and avoid over booking. Please note depending on the date of your event full payment maybe required and you will be informed immediately. All orders payable in full 2 weeks prior to event.
Upon receipt of your form, you will receive an email confirming your payment and any additional information that is necessary. If you have submitted photo(s) your design proof ($15) will be created and emailed to you for approval with payment request/schedule to proceed with order. Please contact me immediately with any questions and/or concerns that you might have.
Please note, I reserve the right to refuse any order request if I deem necessary. All rights of designs are created and owned by Kina's Kreations and permission must be granted for duplication.
